Jackie “Jack” Lee Bolen, age 56, of New Haven, Ohio, passed away on Tuesday, July 18, 2023.  He was born on April 13, 1967, in Willard, Ohio, to the late David L. Bolen and Veronda Kay (Tuttle) Stumbo.  Jack was a 1986 graduate of Willard High School and had retired from Dutch Maid as a truck driver.  He was known for being D.J. Spaz-O-Taz, enjoyed gardening and computers, but most of all, loved spending time with his family.

Jack is survived by his son, Jacob Bolen; brothers Tony “Bones” Bolen of Shelby, Ohio, and Steven (Jessica) Bolen of Plymouth, Ohio; fiancé Jennifer Sword of New Haven, Ohio; children, Sommar Sword, Troy Anthony Lee Sword, Ashton Sword and Emalee Sword; and many aunts, uncles, nieces, nephews, great nieces and great nephews.

In addition to his parents, he is preceded in death by grandparents Feris and Verniece Tuttle.

Per Jack’s wishes, there will be no funeral services at this time.  There will be a Celebration of Life and a burial at a later date at Mt. I was so shocked when I heard of his passing. I'd commented on his posting after he had his heart attack and since he made it thru that, I thought all would be just fine with doctors fixing any issues he had. I'd talked to him recently as well because he was concerned about the lack of bookings he was getting as a DJ and wondered if I was having the same issue myself. Things can sure take a quick turn in life. He was way to young to have left this relm for sure.
